From the book
- Back channel
- A private route that reopens a decision outside the assigned owner's authority. Escalating to align goals with the owner is coordination; overruling the owner without bringing them in is a bypass (Introduction; Chapter 3).
- Decision rights
- The decisions a person may make without further approval, with their boundaries and escalation conditions. Authority is visible decision rights, not a title; ownership without authority is theater (Chapters 1 and 8).
- HEAR
- Honesty, Empathy, Accountability, Respect. The values that set the standard for how a team handles disagreement; the operating system gives that standard a route the person being challenged cannot block (Chapter 3).
- Hero
- The person a company has learned to wait for. The dependency, not the hero, is the disease, and the operating system often helped build it (Introduction).
- Hero Test
- Who is the company waiting for? Why does it wait? What would it take to move without them? Take it on yourself first, whatever seat you hold; it examines a dependency, not a colleague's character (Introduction; Chapter 1; each chapter’s closing section).
- Key-person dependency
- An important result that repeatedly depends on one person's knowledge, permission, relationships, or rescue work. Expertise is not the cause; the cause is resources, authority, or support never granted, knowledge never transferred, or work kept close (Introduction).
- Multiplier
- A leader or expert whose time raises other people’s judgment and output through standards, coaching, teaching, capital, and the relationships and choices that still require their role (Introduction).
- Outcome (versus output)
- The business or customer result the work is intended to produce. An output is the thing delivered; delivery alone does not establish the outcome (Chapter 7).
- Shadow operating system
- The unwritten rule a company actually runs on, whatever the org chart says, and it usually reduces to this: when in doubt, go back to the leader. It slows every decision that could have closed lower and follows the leader to the next company (Chapter 1).
- Shared backbone
- The minimum common standards, controls, information, and interfaces that let work cross a boundary, with local decision authority preserved everywhere else. A standard kept so the center feels in control is bureaucracy, not backbone (Chapter 10).
- Two Laws of Execution
- Decisions Before Data means naming the decision, the options, and the evidence that could change it before collecting more numbers (Chapter 11). Commitments Before Scale means agreeing on the production owner, adoption plan, funding conditions, and the result that justifies expansion before a pilot begins (Chapter 12).
- VOOCS
- Vision, Outcomes, Ownership, Cadence, Systems. The operating system behind every chapter, run as one connected loop. Chapter 5 shows a worked example; the online course adds tools and working sessions (Introduction; Chapter 5).
A1Module 1
Open Module 1- Hero Test
- The diagnostic, run on yourself first: who is the company waiting for, why, and what would it take to move without them.
- Bottleneck Check
- Three questions sorting what waits for the top, for a person instead of a rule, or above the accountable person.
- 48-Hour Decision Queue Audit
- Two working days recording every decision that reaches the person the company waits for.
- Waiting Decision Log
- The audit's record continued: each waiting decision, its age, owner, decider, and deadline.
A2Module 2
Open Module 2- Vision and Priorities
- The result, who benefits, the period, three ranked priorities, and what will not be funded.
- Priority Stack
- The ranked list; the authorized resource owner confirms which priority wins the same people or money.
- Definition Audit
- One term, its meaning in each function, qualifying and disqualifying evidence, and the shared definition.
- Alignment Translation Worksheet
- Each function's contribution, outcome, work to stop, decisions, dependencies, and review date.
A3Module 3
Open Module 3- Backchannel and Bypass Record
- A decision reopened privately: what happened, who had authority to change it, where the final decision lives.
- Tradeoff Ranking
- The Priority Stack applied to one contested choice: rank outcomes, show the scarce resource, record what changes.
- Decision Rights Record
- Who owns, advises, approves and why, resolves disputes, records the decision, and where teams find it.
- Truth Route
- The protected route for suspected misconduct, retaliation, or serious risk, including concerns about the assigned leader, separate from execution disputes.
- HEAR questions
- Honesty, Empathy, Accountability, Respect: the fact avoided, the pressure missed, who acts by when, whose experience was dismissed.
A4Module 4
Open Module 4- Workflow Repair Map
- One real transaction from request to accepted result, with every handoff, delay, control, and proposed repair.
- Operations First AI Gate
- The pre-investment decision after repair: bounded experiment, further repair, or no investment.
- AI Adoption Cadence
- Reviews of actual use and results with users; training attendance and licenses do not establish adoption.
- AI Stop Rule
- The condition that pauses use, who can stop it, the safe fallback, and the evidence to restart.
A5Module 5
Open Module 5- VOOCS Operating Record
- The one record every module writes into: Vision, Outcomes, Ownership, Cadence, Systems for one initiative.
- Hero-to-Multiplier Map
- Where the leader's time goes as decisions leave the inbox: the rescue, its cause, the new owner, the higher-value work instead.
A6Module 6
Open Module 6- No List
- What stops or is declined, the priority it protects, where the capacity goes, and what could reopen it.
- Two-Future Test
- When old and new both demand protection: the future chosen, the overlap cost, milestones, and the end condition.
- Exception Ledger
- Every exception granted in the agreed forum, with approver, cost, displaced commitment, precedent, and expiration.
- Sequence Conversion
- What stops or changes for each affected site, product, or workflow, with owner and acceptance evidence.
A7Module 7
Open Module 7- Done Test (the Customer-Backward Definition of Done)
- Agreed with the recipient before work is called done: outcome, target, deadline, acceptance conditions, evidence, verifier.
- Forward Commitment Dashboard
- The next observable commitment paired with the latest measured result.
- Outcome Closure Agenda
- What was promised, what evidence arrived, what remains unmet, what decision is needed, who owns the next commitment.
- System Outcome Verification Gate
- The receiving function proves the result works in its workflow: data, permissions, support, exceptions, recovery.
A8Module 8
Open Module 8- Authority Grant
- The five elements in writing: decisions, budget and staffing, tradeoffs, standards, escalation with a deadline; for one owner or a unit.
- Required Voice Matrix
- Whose knowledge the decision needs, input distinguished from approval and ownership, with dates.
- Integration Owner Charter
- For work crossing functions: shared outcome, integration owner, dependency handoffs, tradeoff authority, reserved decisions, sponsor.
- Override Log
- Material overrides, those that moved the target, date, resources, or risk, with reason and whether the grant needs revision.
A9Module 9
Open Module 9- Cost of Waiting Calculator
- Acting and waiting compared over the same period, with assumptions, ranges, and a decision deadline.
- Option Loss Review
- What becomes unavailable if the company waits, and the latest useful decision date.
- Delay and Legacy Record
- What delay creates and what makes change costly, in one record; closes in close, escalate, or defer.
A10Module 10
Open Module 10- Backbone and Autonomy Matrix
- For one capability: common, local, or joint, with reason, owner, required control, and review date.
- Reciprocal Integration Plan
- What the acquired business adopts and what the parent provides, removes, or learns, with owners on both sides.
- Key-Person Dependency Map (manager-held)
- One critical capability, its holder, backup, funded practice assignment, and evidence the backup can perform.
A11Module 11
Open Module 11- Decision Brief
- Written before the dashboard: question, options including waiting, evidence, threshold, reversibility, cost of error, deadline.
- Metric-to-Decision Map
- Each measure tied to the decision it informs, its threshold, the authorized response, and its retire date.
- Customer Recovery Evidence
- After the product performs under pressure: did the result hold, what risks a repeat, who owns follow-up.
A12Module 12
Open Module 12- Commitments Before Scale Canvas
- The pilot's front page: outcome, riskiest assumption, thresholds, production owner, capacity, funding path, controls, gate date, decider.
- Executive Adoption Contract
- The leader, the workflow they will use or inspect, expected behavior, evidence, and the response when old habits return.
- Implementation Governance Charter
- Users, controls, customization limit, readiness criteria, training and support owners, exception authority, rollout sequence.
- Production Workflow Map
- Each step's sender, receiver, control or human decision, acceptance evidence, and failure route.

A13The 90-Day Install
Open the 90-Day Install- The 90-Day Install
- One initiative on one record, facilitator-run: a two hour first session, four windows, five review questions, a gate decision.
Roles
- Outcome owner
- Accountable for the result; obtains a binding decision when authority runs out; never the facilitator.
- Sponsor
- Grants and backs the authority the work needs and resolves reserved decisions.
- Receiving owner
- The person who will use the result, or who stands in for a customer who cannot attend.
- Dependency owner
- Commits to deliver what the outcome depends on, with a turnaround the owner can plan around.
- Facilitator
- An operating leader from another function or an outside advisor who runs the room and routes protected concerns.
- Recorder
- Keeps the VOOCS Operating Record during the session.
